A T shape might look good, wouldn't cost you much to extend it...high stools, you could extend in the cabinet wood colour...quick mock-up for you to give you an idea of what it would look like, cheers! eys

The 2nd picture you posted is really small so hard to see but it looks like there is not much room to extend the island/counter. If the island is not fixed to the floor you could bring it out abit towards the railing and towards the patio doors. Then make a new counter that extends past the island/cupboard on three sides so you could put a stool so your back is to the patio doors, a stool on the opposite side so you are facing the patio doors and two stools so your back is to the family room. Or just extend it on the long side and have only 2 stools with your backs facing the family room. Not sure if this is helpful or not.

The kitchen has railings that overlook the family room which we like so I wouldn't want to close or cover that up. I was thinking maybe something like an L shape out from the island. You would still have room to fit maybe 4 chairs and to walk around it. Or is there a better way to make the island a combo island/bar stool area?
Instead of extending the island, another idea is to do more of a lounge area. A couple of comfy chairs with a table in between them or a rattan/wicker love seat could work. We can't see the whole layout of your kitchen - would you extend it towards the door area? Would that leave enough room to go around? Or would you extend it out so it has an overhang towards the table area? How wide would it be then? Could you get 2 stools at the counter? If you can post another picture with more of a view of the whole room, we could get a better picture.
